HOLD AYLUS HARMLESS AGREEMENT

Any member/parent/guardian of the AYLUS, shall indemnify the AYLUS, its board of directors, national and branch officers and advisors, and save them harmless from and against all claims, actions, damages, liability and expenses in from, related to or in connection with any volunteering activities that either members self-starting or AYLUS organized, effectively immediately and until further notice. If the above-named indemnified shall be made a party to any litigation commenced by or against person/organization, then person/company shall protect and hold the above-named indemnified in connection with such litigation.

All participants consent to being photographed and/or videotaped while on volunteering activities for promotional use by AYLUS and/or AYLUS authorized entities.

AYLUS Volunteer Service Report Template

Advisers should use the following Microsoft Excel template to submit service hours for students. Each student should have a dedicated file. Guideline and instructions are included in the template.

For each student who wants to apply for PVSA award, the following application form is also needed by AYLF. The form mainly informs us about the student’s age, home address, etc.

In summary, for a student to receive PVSA award, two files are needed as detailed above. Branch advisers should review such information to the best knowledge to ensure that the submissions bears creditable fidelity and accuracy, summarize all applicants from each branch, then send the compiled application (a spreadsheet) to aylfus@gmail.com.
